How to sign up:

If your school/district has opted into the Covid Weekly Screening program, you will find a consent form below. Click on your school and then your language. Fill out the consent form for each student you would like to sign up.

How to register in Labdash.net:

Labdash.net is our online reporting portal. If the student is under 16, their parent or legal guardian will have to sign up with a patient account for themselves, and then add the student as a dependent, under the parent’s  account. If the student is 16 or older, they can register for their own account.

Welcome to the K-12 COVID-19 Screening Program

Thank you for signing up for the K-12 COVID-19 screening program. You are helping your student, their classmates, and teachers be safe and stop the spread of COVID-19.

What Happens Next?

After your child is signed up through their schools Smartsheet link, you will receive a
welcome email from the scope lab containing:

  • Instructions on how to register for Labdash, our online reporting portal.
  • Instructions on how to collect a spit sample for testing.
  • Spit collection kit (spit sample tube, funnel, etc.) and patient labels.
  • Information on what day of the week to turn it in and where to drop it off.

How Will My Child’s Spit Sample Be Collected?

Each week, you will collect your child’s spit sample. These samples will be collected at home.

  • We will mail you several weeks of collection kits.
  • Your school will provide 4 weeks of collection kits.
  • Follow the included instructions to physically collect your child’s spit sample and then collect the sample into their account on Labdash.net before dropping the sample off at the schools drop-off box on or before the pick-up day for your student’s school.
  • Have your child drop off the spit sample at your school’s collection site.

What Happens to My Child’s Spit Sample?

Your child’s spit sample will be collected by a courier from the Santiam Scope Lab and then delivered to the lab for PCR testing.

How and When Will I Get My Student’s Test Results?

You will get an email from labdash.net notifying you that the student’s results are ready
to be viewed in the labdash.net portal within 24 hours of the samples receipt at the lab

What Do The Test Results Mean?

  • If the results say “Postive for Covid-19”, the Scope lab DID find the virus in your student’s sample.
  • If the results say “not detected”, the Scope lab did NOT find the virus in your student’s sample.
  • If the results say “inconclusive”, the Scope lab was unable to confirm the test results or the sample was too contaminated with food or other substances to be run.

 

What If My Child Tests Positive for COVID-19?

If your child’s test is positive, you will receive a direct phone call from the lab with instructions on what to do next.

What Happens Next?

After you’ve completed the consent form for your student, we will mail the following materials to the mailing address provided:

  • Instructions on how to register for LabDash, our online reporting portal.
  • Instructions on how to collect a spit sample for testing.
  • Spit collection kit (spit sample tube, funnel, etc.) and patient labels.
  • Information on what day of the week to turn the sample in and where to drop it off.

How Will My Child’s Spit Sample Be Collected?

Each week, you will collect your student’s spit sample. These samples will be collected at home.

  • We will provide twelve weeks of collection kits.
  • Follow the included instructions to physically collect your student’s spit sample and then “collect” the sample into their account on Labdash.net. We recommend doing this in the morning, before school, on the drop-off day or the evening before.
  • Have your child drop off the sample at your school’s collection site.
  • If you have a young student or a student with disabilities that has a hard time collecting their saliva sample, please contact us at the lab and we will be happy to talk with you about your options. 

What Happens to My Child’s Spit Sample?

Your student’s spit sample will be collected by a Santiam Hospital Lab courier and then delivered to the lab for PCR testing.

How and When Will I Get My Student’s Test Results?

You will recieve and email notifying you that your student’s test results is ready to be viewed in their LabDash account within 24 hours of the samples arrival at the lab. You can also opt-in to text message alerts when creating your LabDash account.

What Do The Test Results Mean?

  • If the results say “Postive for Covid-19”, the Scope lab DID find the virus in your student’s sample.
  • If the results say “not detected”, the Scope lab did NOT find the virus in your student’s sample.
  • If the results say “inconclusive”, the Scope lab was unable to confirm the test result, or the sample was too contaminated with food or other substances to be run. If this happens you are welcome to submit another sample for testing.

If your child’s test is positive, you will receive a direct phone call from the lab with instructions on what to do next.
